Your Consultation and what to bring:
New patients are asked to arrive 5 minutes early for their appointment to complete paperwork prior to their consultation.
You may download your registration form here, which you can email to [email protected] or bring with you to your appointment.
You will need a referral from your GP so that you can claim your rebate from Medicare, please make sure you bring your referral along with you to you consultation.
If you have any results and/or ultrasounds, please bring them to your consultation.
If you take medications a list is very useful to bring to your consultation.
Payment at the time of consultation is required and we accept Visa, Mastercard, Cash or Cheque.
If you are unable to attend your appointment please ring: (03) 9592 2967. Your telephone call will allow us to minimise inconvenience to you and other patients.
Appointments are prioritised as follows:
- Emergencies
- Patients who arrive on time
- Patients who arrive late
- Non-emergencies with no appointment.
