Your Consultation and what to bring:
New patients are asked to arrive 5 minutes early for their appointment to complete paperwork prior to their consultation.
You may download your registration form here, which you can email to reception@drmanjuagarwal.com.au or bring with you to your appointment.
You will need a referral from your GP so that you can claim your rebate from Medicare, please make sure you bring your referral along with you to you consultation.
If you have any results and/or ultrasounds, please bring them to your consultation.
If you take medications a list is very useful to bring to your consultation.
Payment at the time of consultation is required and we accept Visa, Mastercard, Cash or Cheque.

Emergencies
Patients who arrive on time
Patients who arrive late
Non-emergencies with no appointment.
